All pathways are delivered in a patient’s usual home environment. RR focusses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment and treatments up to 5 days. The DTA pathway aims to support the discharge of patients from acute or community hospital wards to assess and manage rehabilitation/therapy needs, at the point that the patient is medically well enough to go home. EIV is a joint pathway, delivered with Herts County Counsel delivering crisis management to patients offering them a holistic assessment - physical, functional, and social all in one assessment. In addition, the Unplanned Care team provide a same day response to meet urgent and essential nursing care needs of patients known to planned care and providing an additional unscheduled intervention which requires delivery within 24 hours and could not have been anticipated.
