Herts Valleys Adult Community Services dedicated resources to the Unplanned Care team in 2020. The team comprises three pathways delivered over 7-day working, Rapid Response (RR), Early Intervention Vehicle and Discharge to Assess (DTA). All pathways are delivered in a patient’s usual home environment. RR focusses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment and treatments up to 5 days. The DTA pathway aims to support the discharge of patients from acute or community hospital wards to assess and manage rehabilitation/therapy needs, at the point that the patient is medically well enough to go home. EIV is a joint pathway, delivered with Herts County Counsel delivering crisis management to patients offering them a holistic assessment - physical, functional, and social all in one assessment. In addition, the Unplanned Care team provide a same day response to meet urgent and essential nursing care needs of patients known to planned care and providing an additional unscheduled intervention which requires delivery within 24 hours and could not have been anticipated. All pathways are delivered over the 4 localities in West Hertfordshire. All pathways are delivered over the 4 localities in West Hertfordshire: Dacorum, Watford and Three Rivers, Hertsmere and St Albans and Harpenden, where you will be required to cross cover.

We are looking for forward thinking, passionate Nursing Associates (ANA’s will be considered if you are on the course) to support the management of patients within the South and West Hertfordshire localities when they have an unscheduled visit requirement.

System working with our acute hospitals and social care will be a key aspect to the delivery of this service which will generate the opportunity for you to develop skills in system working and service development.

The role requires you to be able to keep accurate clinical records and have good clerical/ administrative skills.

This post offers a great opportunity for carers to develop their clinical skills and have experience working in the community. The organisation is supportive of training needs and there is an excellent mandatory training and education programme. You will also receive regular clinical supervision and safeguarding supervision.

This is an exciting post, with the opportunity to help develop the service and make a real difference to patients and a chance to develop both clinical and strategic skills.

Possession of a valid UK driving license and use of a car for work purposes is essential.
