**Role Description**

We are looking for a **Dementia Activities Coordinator** to work with us with **28 hours per week** based within Colchester Hospital with some potential work at Age Well East's office locations in Northeast Essex. In return, you will receive a **competitive salary of FTE £22,050 per annum (pro-rated £16,464pa).**

**The Dementia Activities Coordinator (Sundowning Project) Role**

You will provide effective wellbeing support for patients living with dementia designed to help manage the onset of sundowning during their stay as in-patients on wards in Colchester Hospital. You will work with patients and their carers (if appropriate) to support them to complete the ‘this is me’ personal record to aid communication with clinical staff and develop and deliver a personal wellbeing and supported activity programme to help identify activities effective for managing potential triggers for symptoms associated with sundowning. The aim of this work is also to support successful hospital discharge and resettlement at home by enabling the patient and their carers (if appropriate) to take away a plan of activities to undertake to at home to continue to manage the onset of sundowning.

**Main Duties & Responsibilities**
- Establish and develop positive and effective relationships with ward staff across Colchester Hospital to support the delivery of the Sundowning project.
- Support patients and their carers (if appropriate) to complete and communicate the ‘this is me’ personal record to strengthen person centred care planning and treatment within Colchester Hospital.
- Develop and deliver patient led wellbeing and supported activity programmes to meet the needs for mental and or physical stimulation within the hospital ward.
- Work with patients and their carers (if appropriate) to develop a supporting plan of activities to undertake at home following hospital discharge.
- Signpost patients and their carers (if appropriate) towards relevant community services to strengthen post discharge wellbeing.

**What we are looking for**

**Essential**:

- Demonstrable in-depth knowledge and experience of Dementia.
- Professional qualifications or training relevant to Dementia.
- Experience of developing and delivering activities effective for strengthen physical and mental wellbeing.
- Experience of managing difficult situations and conversations with people.
- Experience of or knowledge in delivering services within a clinical setting.
- A full driving licence and access to a vehicle.
- Good communication skills and the ability to juggle a range of priorities well.
- Ability to work independently and as part of a team.
- Experience of building and maintaining relationships with key stakeholders.
- Able to understand and promote empowerment and support people in later years to make decisions to help them to lead an independent and fulfilling life.
- Able to understand the confidential nature of our work and put in place relevant safeguarding procedures.

**Desirable**:

- Education or training relevant to working with the third sector and voluntary organisations.
