What is the job role?
We are looking a Community Case Worker to work within our Promoting Independence Team (PI). The PI team is a team of Occupational Therapists, Social Workers and Community Case Workers who work collaboratively to achieve the best outcomes for service users. We work with a variety of different people, from younger adults aged 18+ all the way through to older adults with a variety of medical conditions and disabilities.

As a Community Case Worker you will be completing assessments for ongoing long term care, after the services users period of assessment / enablement.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

Who are we looking for?
You will need to have some experience working within a health and/or social care setting.

A knowledge of the Care Act 2014 and Mental Capacity Act 2005 would be of benefit.

You will have excellent communication, time management and assessment skills.

You will be committed to achieving the best for the people of Coventry. You will be joining a great team of people who are passionate about the city, its people, and its exciting prospects.
