**About the Service - Housing & Homelessness Services**
A safe home is the solid base from which you build your life. We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.
Accommodation to us doesn’t just mean a roof over your head and shelter from the elements. It needs to be a welcoming and flourishing environment, offering opportunities for all those we support to feel comfortable and develop. **About the Job**
This position forms part of the complex needs team working under the Making Every Adult Matter (MEAM) remit supporting individuals facing multiple needs and exclusion.
Working in partnership with stakeholders the post holder will identify individuals and coordinate a housing first programme.
This is a new role to support the partnership between City of York Council, TEWV Mental Health Trust and Changing lives. The primary aim of the project is to support individuals with a long history of mental ill health and living challenging lives to live independently in tailored accommodation setting appropriate to their individual needs.
