Our client is looking for flexibility, enthusiasm, and dedication to working positively with individuals in the community and hospital settings. You will need to be able to form and develop positive relationships with individuals and carers to ensure that theyare supported to achieve outcomes that matter to them.
**Your new role**
You will meet the social care needs of older people, people with physical and learning disabilities and mental health needs in the south of Monmouth.
To develop and deliver high quality social work in accordance with statutory and service area requirements. Develop support plans with individuals and others built upon self-reliance in a positive and empowering way. Demonstrate effective interagency, multidisciplinaryand partnership working to provide a best practice service. You will be managing an individual case load and associated administrative tasks in an efficient and effective manner, using IT systems and software as appropriate. Ensure the implementation of agreedcare and support plans in liaison with other agencies and service providers, regularly monitoring, reviewing and revising as appropriate.
Their aim is to work proactively with individuals, to promote choice, control, positive risk taking to achieve positive outcomes in areas that matters most to them.
**What you'll need to succeed**:

- DipSW /CQSW /CSS/Degree in Social Work
- Registered with Social Care Wales
- Good working knowledge of legislation/policies relevant to Adult Social Care, Adult Protection and Carers (Essential)
- Experience of assessment, 'what matters' conversations and outcomes focused practice in relation to current legislation.
- Experience in carrying out multi-disciplinary assessments of need
- Comply with and support others to observe Health and Safety Act 2009 procedures and processes
