JOB TITLE: Support Worker

REPORTING TO: Home Manager /Care coordinator

JOB PURPOSE: To assist the Care Coordinator/ Home Manager in the effective coordination of a wide range of services and support required by individual Service Users to enable them to live active, fulfilled lives and to maintain their rights of autonomy, attainment, citizenship, individuality, wellbeing, inclusion and diversity.

Required: Genuine interest in and experience of working in a dementia setting with adults for at least one year QCF Level 2 in Health & Social Care Ability to communicate effectively at all levels and know how to use a computer. Satisfactory DBS check. To be registered or willing to register with Social Care Wales.

Desired: QCF Level 3 in Health & Social Care.

1. To take Key Worker responsibilities for an allocated group of Service Users including assisting in the development, delivery and evaluation of person centered programs of care tailored to individual needs and choices with full involvement of stakeholders and the multi-disciplinary team.

2. To supervise individuals within the home, ensuring that they are fully involved and participate in both internal and external activities that promote their individual rights. To promote their Individuality, Choice, Independence, Dignity, Privacy and Respect in accordance with Care Council for Wales.

3. Where absolutely necessary to assist Service Users in aspects of their personal care needs with particular attention to enabling Service Users who have limited mobility, making the best use of aids provided.

4. To assist Service Users in the development and maintenance of practical life skills such as those used to engage with social and leisure activities, shopping, meal preparation and using public transport. This also includes encouraging self-care and wellbeing as much as possible to maximise Service Users ability to be independent.

5. To participate in creating, maintaining and taking full advantage of opportunities for recreation and socialisation to promote fulfilled lives. Support Worker

6. To support Service Users with a range of activities as detailed in their daily living care plan, to ensure activities are set so that people working at different levels are able to participate and interact with their peers, to ensure a positive outcome for all involved. To ensure quality of life and Service Users well being is priority when both planning and carrying out daily living and social activities.

7. To promote Service Users “voice” and facilitate self-advocacy where required.

8. Carry out regular checks of Service Users at intervals determined by senior Staff Members. 9. Make and change beds, ensuring that rooms are clean and tidy, and commodes are empty, in line with the Homes disposal of waste policy. Ensure the Homes resources are used appropriately.

10. Clean and maintain equipment used by Service Users / Relatives e.g. wheelchairs, hearing aids, spectacles etc. Ensure the Home is kept clean and tidy, in line with the Homes attention to detail philosophy.

11. If applicable, care for Service Users clothing and rooms as named, and ensure that all clothing is recorded, clearly marked and that Service User is given the choice of clothing they wish to wear.

12. Assist Service Users who need help during meal times (ensuring awareness of swallowing difficulties, dietary requirements etc). Assist with serving of food / drinks as requested / required. Give the Service User choices of meals where required.

13. To administer medication where this is necessary according to the agreed care plan and as directed by the Home Manager and homes medication policy via Boots E Mar.

14. Answer nurse call system, giving assistance as required and as trained.

15. Escort Service Users if requested, travelling to and from the Home e.g. on social outings, hospital visits etc.

16. To undertake any specific tasks / responsibilities regarding Service Users or the Home as delegated by Home Manager.

17. Make visitors feel welcome and check identification as per the Visitor Etiquette Policy.

18. Ensure full privacy and dignity is maintained for the dying and the bereaved, in line with the Homes policies and procedures.

19. To promote team work at all times.

20.To establish and maintain partnership working via effective communication with any significant other in the Service Users life including spouse, relatives and advocates.
