Learning Disability Care Coordinator

You will be responsible for providing support to practices to ensure that patients receive regular check ups, medication reviews and Care plans. The care coordinator will ensure that the persons needs are met, will value the important of their choice and decision making in their life, and will actively seek to encourage gifts and abilities. The position is available immediately once a suitable candidate has been appointed, full induction will be provided to ensure mandatory training completed and an understanding of general practice. The post holder will need to be motivated to develop the role based on the needs of the practices across the PCN, ideally creating a pathway to ensure all patients receive a high level of support to meet their health needs.
