As a Assessing & Reviewing Officer for Right at Home Sefton you will play an integral part in managing the daily communications between our Clients, Care Assistants, Care Co-ordinators and the Registered Manager. You must be extremely personable as you will be working with the entire team to make sure all our valued Clients needs are met. This role is extremely fast paced and requires someone truly dedicated and able to remain positive and professional when working under pressure.

**Duties **include**:

- Conduct new and existing client assessments as required and produce new support plans and risk assessments in liaison with the Registered Manager.
- To help manage new client enquiries: tracking each enquiry; assessing capacity in liaison with the Registered Manager and Care Co-ordinator(s); arranging assessments where appropriate in liaison with the Registered Manager; ensuring that all new packages which we undertake begin in line with our policies and exceed the expectations of our clients.
- To manage client relationships throughout the term of their service agreement ensuring: that communication with clients from our staff is of the highest standard; that we are as responsive as possible to the needs of the clients; that records of our staff/client interaction are maintained to build accurate historical data of each client.
- To arrange and manage the client review programme ensuring that our policies are upheld as is the quality of our services to our clients.
- To conduct periodic reviews as required with clients to ascertain satisfaction with service and determine any required changes to care plans or risk assessments.
- Participate in our On-call service, enabling the Care team and clients to feel supported outside of traditional Office hours in emergency situations.

We cover Sefton, Formby, Maghull, Crosby, Waterloo, Bootle, Litherland, Seaforth, Netherton, Lydiate, Hightown, Freshfield, Ainsdale, Woodvale, Birkdale, Hillside & Southport, therefore local area knowledge is highly desirable.

**Requirements of an Assessing & Reviewing Officer**:

- You must hold a valid driving licence and access to a reliable car - Essential
- Experience within a health care environment
- Relevant qualification, such as Health and Social Care NVQ or Care Certificate
- Excellent customer service, communication and interpersonal skills
- Good computer skills including proficient use of MS Office, the ability to produce reports and good computer systems knowledge
- Self-motivated and flexible, with a willingness to participate in an on-call system for out of office hours
- Extremely well organised, excellent planning and prioritising ability with high attention to detail
- Ability to establish and maintain effective professional working relationships
- Strong administration skills and ability to manage multiple work loads
