A vacancy has become available for a Band 4 Assistant Practitioner within the Community Physiotherapy service in North East Lincolnshire.

This position is based in the community therapy team with a range of other occupational therapy and physiotherapy staff supporting individuals to remain safe and as independent as possible in their own homes. The post holder will work independently with allocated clients referred to the service, once they have completed the relevant competencies to do so. The caseload of clients treated will have been triaged by registered practitioners prior to allocation. They will also work jointly with the therapists or other assistants to provide interventions with clients who have more complex needs. They will provide physiotherapy assessment and intervention for a range of patients and abilities, providing support with mobility, balance, strength and function. Interventions can include rehabilitation, equipment and advice and education. They will work alongside the other members of the multidisciplinary team.

This team sees patients with a range of different presentations in the community, in their own homes, and Care homes. Your role will involve assessment and treatment using appropriate intervention and rehabilitation for these individuals to improve their abilities, mobility and aid safe independent function in their homes.

The focus of the role is to assess and support individuals to facilitate safety and function in their home environment. Interventions can therefore include a range of assessments and rehabilitation activities, assistive equipment, advice and information sharing.

Excellent communication skills are required to ensure a collaborative approach to patient care from the multidisciplinary team involved.

The purpose of the post is to provide a high standard of therapy service working under the supervision of registered Occupational and Physiotherapists.
