An exciting opportunity has arisen for a Social Work role within a Project that will be focussing on NHS Continuing Healthcare reviews.

Good assessment skills and an ability to identify what care needs fall within the responsibility of the Local Authority will be required for this role.

You will work within an MDT approach, undertaking Social Care Assessments and undertaking Decision Support Tools for individuals the NHS and Local Authority have identified as requiring a review of CHC Eligibility.

Social Workers will be working from home for the majority of the time, however visits to individuals will be throughout Lancashire.

It is essential that you have experience working with Liquid Logic as the Project does not afford time for training beforehand. This post is initially for 10 - 12 weeks, however there may be a possibility for further work.
