Permanent, 37 hours per week.
This is an exciting opportunity for a **Assessment and Review Coordinator **to join our **Transitions Team** to ensure young people in Rotherham receive the right support at the right time: Evidence and research shows that good transitions are essential to help avoid young people feeling like they fall off a cliff at 18 when support from Children's services traditionally ends and they become an adult.
**The Transitions Team is based within Adult Social Care** and has been developed to start working with young people from the age of 14 years to ensure that at 18 the right support is ready for them.
The Transitions Team work closely with Education/ Education Health and Care Plan Team, Children’s Services and Families to ensure the Young Person’s voice remain at the heart of this.
Support to Transition should be ‘aimed at moving a person into work/adult life in such a way as to promote their independence and so reduce their long term needs for care and support’.
To support and lead the Council objective to deliver the Preparing for Adult Outcomes - Health, Independent Living, Community Inclusion/ Relationships and Employment.
You will be joining a well-established team with positive peer support. The team has an excellent range of knowledge and skills. Experience or an interest in working with families who support individuals who present with a learning disability and/or Autism and ADHD is desirable. The team plays a key role in providing a ‘link’ between Adult and Children’s Services; therefore experience/knowledge of working in a multi-disciplinary and multi-agency environment will be needed.
You will need an understanding of: the Care Act 2014 and Adult Safeguarding under Making Safeguarding Personal, Mental Capacity Act 2005, Deprivation of Liberty Safeguards and NHS Continuing Health Care.
