An exciting opportunity for an End of Life Care Facilitator to work within Redbridge Community Health services for NELFT

This post will enable, support and enlist GP practices, community services, and care homes within the Redbridge locality to adopt, implement, sustain and develop local and National End of Life Initiatives. The aim is to improve the quality of care at the end of life for all patients, irrespective of diagnosis, and enable more patients to live and die in the place of their choice.

To provide facilitation to localities to support implementation and monitoring of end of life initiatives and to promote new ways of working and good practice highlighted by The Gold Standard Framework (GSF) and Advance Care Planning (ACP).

To engage and liaise with the Redbridge Specialist Palliative Care Team and Community Teams to take the lead on initiatives on end of life care including the promotion of the end of life care register. To develop a work plan to deliver initiatives and evaluate.

To facilitate GP’s to identify named co-ordinators within their practice who will set up palliative care disease registers, palliative care team meetings (GSF) and the setting up and maintenance of the electronic palliative care register.

Probationary Period

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £3,899 and a maximum payment of £4,968 per annum pro rata).
