**About the role**

An exciting opportunity to be part of our Prevention and Wellbeing Team where you will work with Barnet residents to achieve objectives in remaining independent and accessing community services, alongside supporting local communities to coordinate and grow what is on offer in the local area. Working in a creative, strengths based and personalised way you will support local residents who have had contact with Adult Social Care and their carers to find practical and sustainable solutions to achieve their objectives. Focusing on what already exists within our local communities, coordinating this information, identifying gaps and opportunities, developing new initiatives and encouraging the local community to take the lead on managing this long term. You will build strong relationships with local partners, understanding the value of local residents and providing support to increase accessible community initiatives and promoting an integrated and collaborative approach.What the current Prevention and Wellbeing Coordinators say:“ I enjoy supporting residents to achieve goals that matter to them.
