**Client Engagement and Goal Setting**: Initiate contact with referred clients, assessing eligibility, and aligning them with the Spring service. Conduct goal-setting sessions, ensuring a personalized approach and fostering independence, on either a 1:1 basis or group consultations. You will be using coaching and motivational interviewing techniques to encourage people to make positive changes in their lives based on what matters to them. Demonstrate empathy, connecting effectively with individuals seeking guidance by taking a strength and assets-based approach.

**Service Access and Feedback**: Link people to local community groups and services, empowering them to participate in community groups. Understand, manage, and maintain monthly targets in line with contractual agreements. Outcome Measures and Wellbeing Groups: Proactively manage client caseloads, ensuring timely and effective triage, starts, follow-up appointments, and group facilitation to meet or exceed expected targets for client engagement and outcomes. Give people the opportunity to talk, taking a holistic approach and helping them identify what matters to them to support a personalised plan Facilitate sustainable peer groups related to long-term health conditions.

**Record Keeping**: Maintain timely and accurate records of client interactions, outcomes, and referrals, within the expected timeframe for efficient data management. Promotion and Case Studies: Proactive forging of relationships to promote the Spring service, record all interactions, and seek consent for impactful case studies.
