Responsible for arranging appointments for initial and review assessments on behalf of the case coordinators for patients/clients in respect of their eligibility for NHS Continuing Healthcare (CHC) and NHS-funded Nursing Care (FNC), in a timely manner, and for ensuring that all outcomes have been issued to referrers and clients/representatives.

**The administrator will be responsible for**:
Providing administration to all service functions including panel meetings, ensuring that the appropriate documentation has been provided, liaising with meeting members, facilitation of the decision making process including the taking and transcribing of minutes and all follow up work as a result of CHC and FNC eligibility decision.

Communicating on a daily basis, both verbally and in writing with team colleagues (clinical and non clinical), nursing home staff, other health and social care agencies, patient representatives and the general public, at times providing and receiving sensitive and confidential information.

The delivery of an effective and competent level of clerical support and consistently deliver a “client-focused” service.

Development of good working relationships with care homes, domiciliary care providers, Local Authority colleagues and colleagues from other NHS bodies.

Supporting the procurement process for placements and community care packages from private providers of nursing homes and domiciliary care providers.

Acting as the first point of contact for a wide variety of stakeholders including patients/clients, families and carers, general public, acute and community providers, NHS and Local Authority staff and other external agencies.

Recording of all new referrals in a timely manner on appropriate databases and prioritising referrals as agreed by CHC management.

Management and completion of any ongoing communication arising as part of the referrals process.

Liaising with all relevant parties including clinical staff and local authority staff, private providers, patients/clients, families/representatives to ensure assessmentsand review appointments are conducted within the National Service Framework timescales.

Provision of administrative support including typing bespoke letters, requesting outstanding information, file and diary management.

Contribute to policy development and transformation in response to CHC service reviews, national and or local.

Deliver against objectives, achieving quality outcomes, prioritising own workload and working to deadlines.

Proactively identify opportunities for service development and service improvement.

Develop a culture that promotes equality and values diversity. Accurately input and update data onto relevant database

Responsible for reporting any inaccuracies of data and correcting where appropriate.
