Medway Community Healthcare provide specialist dementia care at Darland House.

You will work as part of a well-established multidisciplinary team supporting residents with a diagnosis of dementia. Ensuring that they are given high quality nursing care. We see the person not the dementia, focusing on patient centred care.

You will develop your skills around mental capacity assessments, DOLS, care plans and management of distressed behaviour.
- Assist the unit manager with setting standards of care by leading and developing the team, using and embedding evidenced based practice.
- Responsible for the day to day running of the unit and ensuring that patient care is optimum at all times
- Responsible for ensuring excellent clinical standards on the unit are met at all times.
- To provide and co-ordinate clinical care and support staff in decision making
- To lead on risk assessment and management with junior staff, escalating where required to senior manager/head of Service or manager on call.
- To assist the Band & team leads in staff rotas, PDRs, sickness absence to ensure safe staffing levels.
- To support continuing health reviews and DOLS assessments.
- To work proactively to promote and ensure holistic care is given.
- To act within professional standards, including record keeping, confidentiality and drug administration and NMC code of conduct

1. Communication and relationship skills
- To lead and ensure the assessment, review and care planning of residents needs is robust and embedded in practice
- Provide expert clinical intervention by promoting timely and appropriate decision making and assessments for residents
- Participate in the writing of reports in an accurate thorough way using IT, electronic documentation
- To actively liaise with other MCH services, acute providers, Continuing Healthcare, Safeguarding, Dols, Community Mental Health
- Work with multi-disciplinary team to facilitate timely and appropriate clinical management of residents.
- Provide clinical leadership and act as role model for all unit staff
- Ability to communicate information regarding the resident’s medical condition and individualised treatment programmes in an easy to understand, reassuring way, enabling and motivating where necessary
- To attend and actively participate in team and service meetings

1. Knowledge, training and experience
- Registered Nurse. Desirable at degree level or Mental Health Nurse with at least one year experience as Band 5.
- Post graduate professional development - related to maintaining registration
- Able to evidence continuous professional development in their current field or adjunctive field
- Proven ability to work on own initiative and accept personal accountability

Below is the usual clinical training and experience required for an Adult nurse working in community health services:

- Preparation of mentor’s qualification
- Obtain and interpret patient’s observations News Score, Restore
- Male and female catheterisation
- Know principles of infection and prevention control
- Verification of death
- Sound clinical knowledge and skills in the support of immediate management of residents requiring clinical intervention
- Experience of multi-disciplinary working
- DOLS

Below are transferrable skills
- Understanding of budget/resource management
- Able to work flexibly and cooperatively as well as autonomously
- Confident with decision making
- Possess a working knowledge of IT software programmes and how they function, with a good standard of inputting skills
- Excellent interpersonal skills
- Negotiation and problem-solving skills.
