CLCA Nursing is seeking a Care Coordinator

Do you have a passion for supporting people and ensuring they receive the best possible care?

We are seeking a highly motivated and detail-oriented Care Coordinator to join our dedicated team. In this key role, you will play a vital part in supporting our clinical staff and ensuring seamless care delivery for our valued clients.

What will you be doing?
- Become the expert in our care packages: You will be assigned to your own packages, getting to grips with the care plan and routines to ensure smooth and patient-focused care to clients.
- Communication is key: You will be the bridge between team members and office staff, keeping everyone informed of updates and changes for seamless continuity of care.
- Supporting staff: Be the first point of contact for staff queries, providing clear and timely answers, especially regarding changes in packages.
- Problem solver: Identify and report any issues with packages, following up to ensure resolution is offered.
- Keeping everyone safe: You will play a crucial role in incident reporting and safeguarding matters.
- Training and Supervision: Track clinical staff training, conduct quarterly and annual supervisions, and act as a point of contact for the senior team.

You will need:

- A strong organisational mind and a keen eye for detail.
- Excellent communication and interpersonal skills.
- The ability to prioritise and manage your workload effectively.
- A passion for providing high-quality care and a commitment to client safety.
- Experience in a care setting is desirable but not essential
- A full UK driving license is desirable but not essential
