We’re hiring a Project Lead to join our new Core20PLUS5project team at Live Well Wakefield, who’ll be supporting local residents to improve their health and wellbeing and working to reduce health inequalities across Wakefield District.

**What is the Core20PLUS5 Social Prescribing project?**

Our Core20PLUS5 project is starting in January 2024 and colleagues from Live Well Wakefield will be working with two local community anchor organisations and a team of fantastic volunteers. Live Well Wakefield isa social prescribing and self-management service that supports adults to improve their health, wellbeing and independence.

Many adults are impacted by social barriers and non-medical needs that impact their ability to take up and attend health care appointments, and to adhere to health care plans. The team of volunteers will support individuals to attend local appointments, and a Social Prescriber will work with those same individuals to address any non-medical needs, working together to improve a persons overall health and wellbeing and reduce social barriers in order for them to be able to take up, attend and adhere to health appointments and care plans independently in the future.

**What will the Project Lead do?**

The Project Lead will oversee the development, implementation, and ongoing evaluation of the project along with Live Well Wakefield management, to ensure the project achieves its aims, and meets the needs of clients and commissioners. This will include:

- Working with the Core20PLUS5 team to develop the project
- Working with stakeholders to identify adults impacted by health inequalities and build referral pathways to encourage referrals into the project
- Leading on the day-to-day operational delivery of the project, including co-ordinating and allocating referrals
- Providing peer advice, and supervisory support some members of the Core20PLUS5 team
- Monitoring project performance against targets

**What are we looking for in our Project Lead?**
- Experience of leading on or being involved in delivery of health and wellbeing projects
- Experience of working with communities of interest, individuals, or groups
- Good knowledge of how health inequalities can impact long-term health and wellbeing
- Excellent interpersonal, communication, and presentation skills
- Ability to recognise, assess and deal with potential risk in relation to client, personal, and colleague safety
- Ability to develop project work with partners
