**Job Description**:
A fantastic opportunity has arisen for a Support Worker to join a Local Charity Organisation to support in their Housing team based in Wiltshire.
- Contract: This is a full-time, permanent role, Monday to Friday (working 1 in 4 week on-call)- Salary of £22,440 - £23,812 per annum (an extra £100 per week when on-call)- Location: Salisbury with travel around Wiltshire- Travel required therefore you will need a valid UK driving license and access to own vehicle - business insurance will be required for roles involving travel for work**The Support Worker role**:
You will provide person-centred, high-quality support and housing management services to single homeless clients with low to medium level needs living in designated properties.
Offering effective support to ensure that clients move-on sustainably to greater independence within defined timescales.

**Key duties include**:

- Carry out the client referral and assessment process- Implement and monitor client support plans and risk assessments- Give clients advice, information and provide an enabling service so that they can be signposted to and access appropriate services.- Maintain client records that are accurate and fully updated using the client record system.**The Successful Application**:

- Previous work with homeless people and/or other socially excluded people.- Ability to undertake assessment and planning for people with complex social needs.- Able to work as part of a team in a fast-paced environment- Able to work on own initiative**To Apply**:
