**About the role**:
Hammersmith & Fulham Council are at an exciting phase of developing our Family Hubs and Start for Life offer. Are you experienced in transformational change and enhancing services for Children and Families?

In this varied role, you will oversee the Start for Life and 5 Years Plus offers within our Family Hubs with a focus on referral pathways for families, managing staff who work with families and in the community as well as developing and coordinating our services for children and families.

You will work as part of a team to understand the needs of families and their communities and to deliver services that address these needs and to develop effective relationships with key partners to support the delivery of Family Hubs.

Transformation of Children's Centres to Family Hubs is a key role and linking with partners both internally and externally to ensure we meet the expectations set by the DFE & DHSC. As such this is an exciting time to be joining our team.

This post will be key in supporting all families including those from our most seldom heard communities/families in increased access to support and community engagement. The role will also support the developments identified from the Early Help System Guide.

This role will require proven experience in supporting families/communities.
