**Location **Carers’ Support East Kent, Innovation House, Discovery Park, Ramsgate Road, Sandwich, Kent CT13 9ND

This role is based in the office at Sandwich. We are helping Carers who are based in East Kent and the post holder may need to occasionally travel across this area to the East Kent hospitals or Carer Support groups or events or Carers homes.

**Salary** £22,650.00 raising to £24.000 after successful completion of probation

Carers’ Support East Kent is a charity that provides Carers with the information and support they need. Our services are available to people looking after a relative or friend, who due to physical or mental illness, age related difficulties, disability or an addiction, cannot manage without their support.

Caring can be rewarding but also challenging. Each situation is different, but we know that caring brings practical, physical and emotional challenges. We care about the health and well-being of Carers and provide information, support and signposting/referral to services to help with those challenges.

We are helping Carers who are based in East Kent.

Our Charity provides a valuable and essential service in East Kent to people who find themselves in a caring role for a friend, neighbour, or family member, who may be suffering from physical or mental illness, addiction, or disability.

Our well-established Charity has been supporting Carers for 20 years. As an organisation we are continually developing our service as we strive to meet the needs of an increasing population of Carers.

Key Responsibilities:

- Provision of a telephone triage, assessment and support service to Carers from our Carers Hub
- Respond appropriately to calls received in the Carers Hub, directing the caller to the most appropriate team member
- Manage incoming referrals, ensuring procedures are followed and information systems are updated
- Provision of emotional support (not counselling) to Carers
- Work with Carers to identify their needs and appropriate support options to meet needs. This could involve visiting any of the hospitals in the East Kent area to talk to Carers.
- Take into account Carers views and feelings in relation to their care, support and treatment, or that of the person they care for
- Empower Carers by supporting them to develop confidence and skills to encourage self-advocacy
- Undertake assessment reviews which could include home visits to Carers to assess their telecare and equipment needs.
- Attend support groups or events to facilitate a session for Carers.
- To help with follow up calls to Carers as part of the Carers Assessment process.
- Liaise with and make referrals to other organisations on behalf of Carers
- Assess Carers’ eligibility to access funded support, such as the Clinical Commissioning Group (CCG) funded Primary Care Rapid Access Service and grants from Carers Trust and other charities
- Arrange and coordinate support services for Carers through the Carers Hospital Discharge Service as required to support Carers Respite Coordinator
- Manage bookings for Carer events and activities and organise transport and respite to facilitate attendance
- Record Carer contact and support information on database and information systems within timeframes set out in procedures
- Provide examples of impact of support to Carers using case studies, written reports and evaluation tools as required
- Contribute to Carers newsletters and promotional materials
- Any other reasonable task requested by the CEO, Managers or Trustees
