**The Role**

NHS Lanarkshire is currently delivering on an ambitious plan to implement new and changed ways of working to improve patient flow within Lanarkshire. This work, that we have called Operation FLOW, spans the full patient pathway, from avoiding hospital admission through discharge and beyond. To ensure that we can establish and sustain these improvements for patients we are investing significantly in our workforce and expanding new and existing roles.

One of these roles is Assistant Practitioner within the Hospital at Home team.

Hospital at Home is a multidisciplinary acute care team, made up of NHS Lanarkshire consultants, advanced assessment nurses, allied health professionals and community psychiatric nurses. As an alternative to hospital, the team deliver specialist, coordinated and comprehensive assessment and care to frailer older adults in their own homes.

To work under the supervision of registered therapy and nursing staff. To assist patients to achieve their rehabilitation goals.

Implement and adapt treatment programmes within hospital and community settings for individuals / groups of patients.

Undertake baseline assessments within clearly defined parameters for the identified service.

Use specialist technical and practical skills to deliver flexible, patient centred programmes.

Duties may vary depending on the care group / department or assigned area of work.

**What we'll need you to bring**
- HNC in healthcare, education or social cares or SVQ III in healthcare, education or social care or equivalent experience in a health, education or social care environment
- Short specialist courses relevant to specific clinical area
- Full driving licence holder
- Awareness of health legislation relevant to the clinical area
- Knowledge of the therapeutic process, techniques and / or activities relevant to client group
- Basic assessment skills
- Effective listening, communication and interpersonal skills
- Time management, planning and organisational skills. - Working knowledge of basic information technology
- Ability to work individually, or as part of a team
- Ability to learn and develop new skills and maintain a record of learning

**Contract type**

Band 4 /** Fixed Term or Secondment for 6 months** / Full time and Part Time / Various hours.

Please note for part time posts, the salary will be pro-rata.

**Location and Working Pattern**

The working pattern for this role is 2pm - 10pm 5 days per week or 8am - 6pm Sat & Sun.
