**Salary: up to £22,400 per annum**

**Location: Shrewsbury, Shropshire**

**Contract/Hours: Permanent/Part Time - 20 hours per week**

**A bit about the role**

We are looking for an Outreach Worker, based from our service Bradbury House, to work in the community of Shropshire, providing support and care to children in their own homes. The working hours for this role will include days, evenings and weekends shifts.

As Outreach Worker you will assist families where there is a child with a physical and or leaning disability to achieve a normal life pattern by providing person centred** care, enabling**, supporting and empowering our young people to participate and engage in hobbies and local community services.

**How you'll help to create brighter futures**
- Providing a high-quality service to children, young people and their families, based on an assessment of need and being outcome driven
- Communicate appropriately with services users, their families, colleagues and other agencies and professionals.
- Support the young people on social activities or outings
- Oversee and assess the development and behaviour of children and young people and contribute to the prevention of challenging behaviour
- Providing personal care and the administering of medication.
- To undertake a relevant NVQ in Care and complete within three years of employment unless formally exempted due to other nationally recognised qualification

**Let's talk about you**
- NVQ Level 3 or equivalent in a related field
- Experience of working in a health, early years, or social care setting.
- Experience of working as part of a team or as an individual to deliver a frontline service
- Ability to undertake home visits
- Ability to engage and work with children/young people and their families, demonstrating a range of communication skills.
- Access to transport is essential to be able to travel between homes in remote locations on a regular basis.
