**About Us**
At Yorhealth we provide nurse-led care for Children and Young Adults with Complex Medical care needs 24 hours a day, 7 days a week.

**About the role**
The role is to support a 18 year old in her family home, working as part of a vibrant team in a happy and friendly family environment. Supporting her with everyday needs as well as working closely with her specialist team.

**About Me**
I am currently 18, I have Global Development Delay, Scoliosis, Canavans disease and Type 1 Diabetes. It is important to me that my team are supportive of my rights as well as working in partnership with my family, being advocates for my best interests. My mum and dad have cared for me my whole life, they know me better than anyone else in the world and they need to be shown respect at all times.
I have a continuous feed and water pump, and have a plan with the schedule for this. I am completely immobile and have the use of a track hoist in my bedroom to help me get in an out of bed or chairs. I love everything to do with music, especially Bollywood, which my dad sings to me daily. I enjoy spending time with all of my family and people generally, I'm a social butterfly.

**About you**:
We are looking for a warm and friendly individual who has a passion for young people's wellbeing and has a kind and caring nature.
We would like for you to have an understanding of the developmental and educational needs of young adult's.
Current clinical competence is advantageous, but not essential as full training is provided.
A Level 3 (NVQ/Diploma) in the relevant field is desirable.
