Therapy representation in the Transfer of Care Hub ToCH is considered highly important in being able to bring ToCH and Therapy teams closer together, and in supporting effective discharge planning within NBT. The role will be based within the ToCH with management from the Head of Integrated Discharge. Particular focus will be on education for hub and therapy teams, support and input for complex discharges from a therapy perspective, liaison with front door teams where expertise is necessary, developing of relationships between ToCH staff and therapies.

**Primary objectives within this role include**:
Enhancing relationships, trust, confidence, understanding, between NBT therapy teams and ToCH partners

Supporting with discharge training within Therapies and ToCH, with potential to extend to other staff groups

Supporting ward teams with risk management related to discharge and therapies

Rolling out of the Therapy discharge training package at NBT, in combination with therapy leads

Working closely with therapy and ToCH team leads to drive optimum discharge performance within NBT through collaborative working, education and support, and continuous improvement

Input and review of patients in ED and admissions units who may be able to have admission avoided through therapy intervention, but who sit outside of other therapy provisions in that area

Collaborative review of patients with NCTR, in-particular those awaiting or navigated to pathway 2, in an attempt to follow Home First principles wherever possible

Engagement with ToCH improvement projects aimed at driving the home first agenda and facilitating discharge at the earliest possible opportunity

Participate in working group aimed at making the ToCH OT role effective

There is peer and senior support available for this post, which is part of the wider occupational therapy service at North Bristol, a large teaching trust that provides a number of specialist regional services. Regular placements are provided for undergraduate students and the role will also include supervision of Band 6, Band 5 and support staff.

Clinical

1. To provide advanced discharge-focussed Therapy advice, intervention, and support to members of the MDT, patients and families, including complex cases (e.g. patients with a wide variety of medical, functional and social problems), demonstrating excellent clinical reasoning.

2. To provide highly specialist therapy assessment of patient’s skills, functional performance needs and need for specialist assistive devices and/or building adaptations. Consideration is given to a patient’s views and wishes, medical history and prognosis, physical skills, sensorimotor skills, communication skills, social skills, cognitive abilities, behaviour, safety issues, environmental and psycho-social factors, and where appropriate, the viewpoints, abilities, and other issues of carers - these multiple factors are often conflicting.

3. To use advanced skills in analysing the results of the assessments, formulate professional judgements, work with patients to formulate discharge plans, which are appropriate for the patient and others involved in their care.

4. To be responsible for deciding priorities for own work area (with direction from the Head of Integrated Discharge), balancing other clinical, non-clinical and professional demands, and to ensure that these remain in accordance with those of the specialty and Trust wide. This may involve reprioritising own and other staff’s work activities and schedules

5. To assess and manage clinical risk within own and team’s caseload, responding to and resolving problems identified by others.

6. To plan and implement interventions related to discharge planning with the patient or with their carer, and with relevant members of the ToCH and MDT, for patients with reduced capacity, in line with Trust policy.

7. To take responsibility for the health, safety and welfare of the patient during any intervention, and considering control of infection and management of medical needs when appropriate.

9. To monitor, evaluate and modify discharge plans for patients with multiple needs in order to work towards appropriate discharge.

10. To undertake risk assessments in relation to patient and carers in order to reduce risks associated with discharge planning, maintain independence and improve safety. Provide advice and develop strategies in relation to discharge outcomes and liaise with other professions/agencies regarding future management.
