We have an exciting opportunity for Reablement Therapy Workers to join our supportive multi-disciplinaryReablement Service within Keynsham.

The service operates 7 days a week throughout the year, between the hours of 8am and 10pm with various shift patterns available (full-time and part-time) such as weekend orevening shifts only which will suit a variety of work/life balance requirements.

Your base will be within our Keynshamoffice and you will travel throughout BANES (mainly the North East Somerset area) visitingservice users in their houses or within care homes.

You will receive a thorough and supportive induction with in-house training and competencies led by our very own Reablement Learning and Development Lead.

The aim of the service is to assist service userswith a range of physical and psychological needs during theirrehabilitation journey, as guided by the qualified therapists. You will provide rehabilitation to patients in their own home to restore independence with functional activities and provide support where this is not possible.

Main duties of the job

The post involves working with people with a range of physical and psychological conditions and diverse needs within the context of a community-based multidisciplinary team.

The post holder will be expected to:

Support clinicians with their assessment and intervention plans Provide rehabilitation and support as may involve delivering equipment items, promoting independence in daily living tasks, including personal care, washing and dressing, meal preparation, general mobility and transfers. Encourage rehabilitation approaches torestore independence To carry out basic nursing care tasks as necessary, including continence management and the prompting of medication To feedback progress of service users, liaising with colleagues and other agencies and professionals in order to provide a co-ordinated service. This might include GPs, district nurses, social workers and family and carers.

You will have experience of working in a care setting care home, hospital, community care or rehabilitation alongside knowledge regarding rehabilitation and the importance of maintaining independence.

Due to the community nature of this post, you will require a Full UK Drivers License and have access to a car / vehicle.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages Outlook, Word, Excel.
