Virtual Ward, Health Care Assistant, Band 3, Full and Part time available

We have a unique opportunity for you to join our virtual ward hub. You will be at the forefront of this exciting new initiative being expanded to incorporate patients with Frailty Syndrome and Heart failure to our existing Respiratory cohort of patients. We are looking for experienced, motivated health care assistants with a passion to deliver exemplary care to patients in their own home as an alternative to hospital based care. Supported by the clinical team including specialists in all branches of the Virtual ward, you will enjoy a varied working day, providing individualised care to patients at acute hospital sites and community settings including the patients own home. You will provide the support and care required to enable patients and their families to reside on a virtual ward including helping them with technology.

You will work alongside the Virtual ward Clinicians to deliver effective and high quality services for patients on the Virtual ward across North Cumbria.

You will have direct contact with patients, their families and carers to deliver personalised monitoring, care and education. You will also participate in remote monitoring and telephone and video assessments. You will provide, receive and process routine information. You will participate in regular MDT meetings.

The Virtual ward team is a highly supportive environment. The clinical team is passionate and enthusiastic about delivering and developing this new service. You will be welcomed for your own experience and skills and we are excited to develop the HCA role for Virtual wards.

The post holder will work under the direction of a registered professional/Person in Charge; they will spend the majority of their time working unsupervised delivering personalised care and nursing duties in the patient’s home, hospital or hub.
