Intermediate Care Service Occupational Therapist

To practice in accordance with the College of Occupational Therapy Code of Professional Conduct and The Health Professions Council Regulations.
To provide quality occupational therapy services to patients in the Intermediate Care Service. To work with a complex caseload undertaking assessment, planning, treatment, and evaluation of patient’s needs, with supervision from senior OT staff.
To work with the Intermediate Care team to ensure the provision of high quality and responsive Occupational therapy services within the Intermediate Care Service, maximising occupational performance in relation to social, physical, psychological, and environmental needs of adults. These needs are often complicated by the multiple pathology of old age.
To contribute to the analysis, planning, audit, development and evaluation of clinical services within the community therapy team.
The role of the Intermediate Care Service is to provide efficient and effective patient focused care to all service users within primary and secondary care settings. This seamless provision ensures equity of service, patient choice and high standards of personalized care.
The role of the Intermediate Care Service is to provide specialised rehabilitation to patients within interim 24hr bedded settings, facilitating smooth transfer from Acute and Community Hospital settings. To assist patients to achieve their optimal level of independence and integrate back into their communities with the appropriate level of support for them and their carers. To prevent inappropriate hospital admissions and promote a supported discharge from hospital as early as is appropriate and safe. To provide high quality multi-disciplinary (MDT) assessment and treatment, advice/support and / or referral onto other agencies.
The referrals will predominantly be from both the Acute and Community Hospitals, but where appropriate from community settings where is has been recognised that a patient needs to step up into a service such as the Intermediate Care Service to prevent an admission to the Acute Trust.

**Enhanced Check**: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.
