**SALARY**: £22,572 - £25,927 per annum (SCP 11-18), pro-rata

This role is part of an exciting new project to improve the physical and mental health of people living in Wokingham with severe mental illness.

The role will aim to engage with people to improve access to physical health checks, and work in a person centred way to improve connections to other relevant services and sources of support within the community.

ABOUT THE ROLE AND CANDIDATE*
Two Physical Health and Wellbeing Workers will form a small team working closely with Primary Care Networks in Wokingham. The key responsibilities of the role are:

- To safely deliver comprehensive physical health check assessments and review appointments for people on the serious mental illness (SMI) register.
- To promote physical health, wellbeing and lifestyle choices based on individual needs to reduce health inequalities and the mortality gap.
- To support service development for physical health and wellbeing

**Key responsibilities may include**:
**Manage physical health reviews for people on the Serious Mental Illness (SMI) register**

- Safely deliver Physical Health Checks (alcohol, blood glucose, blood lipids, blood pressure, BMI weight, smoking) for people on the SMI register in Wokingham
- Complete work via outreach to engage with people who are reluctant to go to clinics for the Physical Health Checks
- Work closely with physical health leads in Community Mental Health Team(s) (CMHT) and Primary Care (PC) services to support individuals with mental health conditions to improve their physical health
- Accurately input and ensure records are maintained on relevant systems in accordance with organisation standards
- Adhere to agreed governance structures, policies, and procedures at all times

**Promote physical health, wellbeing and lifestyle choices based on individual needs to reduce health inequalities and the mortality gap.**

- Provide psychoeducation around the role of routine physical health monitoring in maintaining wellbeing (including to carers, families and relevant professionals)
- Beyond routine physical health monitoring, collaborate with service users to develop, implement and review plans to improve physical health and wellbeing according to individual need
- Refer and signpost service users to appropriate support available via clinical and non-clinical pathways

**Support Service development for physical health and wellbeing**

- Identify gaps in provision and procedures for attaining key physical health outcomes
- Identify opportunities to improve support for service users with the wider physical health/wellbeing agenda
- Develop links with local services such as leisure centres and clubs to enable service users to improve their health and wellbeing within their community

**Supervision, training, meetings and support**

- Attend regular supervision and participate in annual appraisals.
- Be proactive in reviewing and evaluating personal performance and identifying areas for improvement and development
- Attend and participate in regular clinical multidisciplinary team meeting and other meetings as required
- Attend relevant training with Oxfordshire Mind and within Primary Care Networks, as appropriate
