**Main duties and responsibilities**: To provide expert advice and leadership in relation to the management, reporting, and investigation and follow up of adverse clinical incidents. To support fellow Directorate Safety Partners (8a and 7) in the delivery of the patient safety strategy. To support the development and updating of the relevant risk management policies in line with national drivers, legislation and corporate objectives. To ensure that Directorates are supported in utilising the incident reporting system that meets their needs both in the completion of and in using the database to manage their incidents locally.

To actively promote the Trusts Managing and Reporting Incident Policy and Procedures throughout the Trust, as the basis for reporting and mitigating adverse incidents. Play a key role in the co-ordination and delivery of patient safety across corporate departments. To provide support and advice on behalf of the Trust to families who have been affected by an adverse event, which may have resulted in bereavement. To provide support and guidance to staff who have been involved in a serious incident as appropriate.

To promote the reporting of all adverse incidents through a sustained campaign, which emphasises a proactive and just culture of continual learning from mistakes. This includes encouraging the use of Trust electronic systems for reporting incidents, ensuring all information processed for patients and staff is kept confidential, accurate and in line with the Data Protection Act 1998 and Caldicott Policy. To provide support and advice to managers and clinicians dealing with serious adverse incidents. In consultation with the Head of patient Safety, initiate and co-ordinate the investigation and follow-up of serious adverse incidents.

To support the Safety Surveillance Team with national Central Alerting System (CAS). Liaise with appropriate external agencies e.g. Medicines and Healthcare Products Regulatory Agency (MHRA) in the reporting of adverse events. To support the Directorate Safety Partners, the Heads of Patient Experience and members of the Quality Team in improving the experience of our patients and their families To contribute to the delivery of an ongoing programme of training and development to meet the needs of staff in the areas of risk assessment and management, including the training of appropriate personnel in the investigation of serious adverse incidents using the protocol adopted by the Trust in its Managing Incidents Policy To ensure that the training for adverse incident investigators focuses on report writing, analysis of information, time management, presenting reports and closing the loop for assurance To develop and maintain links and partnerships with departments with a key role in managing clinical risk, to ensure the effective exchange and implementation of ideas, information and learning To monitor and support the directorates in managing, closing and learning from actions and incidents To liaise and implement close working with the safeguarding leads and translation of lessons from serious case reviews for active promotion of protecting vulnerable children To support the Directorate Safety Partners (8a) in liaising with appropriate external agencies as required within current legislation e g.

Ionising Radiation (Medical Exposure) Regulations (IRMER) 2000 in the reporting of adverse events To contribute to the clinical risk management and adverse incident information for the Trusts performance review system, Quality, Safety, Outcomes and Compliance Committee (QSOCC) and the Quality Safety and Assurance Committee (QSEAC) To provide aggregated themed risk reports including claims, complaints and incidents. Work with the other Directorate Safety Partners and the Head of Patient Safety to develop routinemonthly adverse incident reports and set up a system to respond to one off requests from the Clinical Units for specific information reports To assist the Directorates to develop action plans and work programmes to reduce the recurrence of incidents To manage the monitoring of the risk registers and access relevant reports provide highlight report to the Head of Patient Safety in relation to key risk areas To provide training on the management of locally owned risk registers, ensuring regular reviews of the risks, the risk grading, the mitigations and plan to ensure the safe management of risks To support the Directorate Safety Partners (8a) in their preparation of Board, Committee and Divisional reports including highlighting relevant current risks that may impact negatively upon patients, relatives, carers the public and the organisation To support the Directorate Safety Partners (8a) and the Head of Patient Safety to prepare for any external accreditation with regard to Risk To develop robust reporting systems in managing new commissioning arrangements for reporting of external events To liaise, in conjunction with the Directorate Safety Partners (8
