The team of ACP's will be the clinical leaders for the virtual wards, overseeing patient admissions, managing the nursing team to ensure all patients receive the care required. The ACP's will be the liaison point with the consultants and acute clinical teams for patient escalation and progression. Between 8pm and 8am the ACP will be the point of contact for patients on the virtual ward and will use the agreed SOP's to escalate care to the relevant acute team when needed.
- Lead daily virtual ward rounds when on shift.
- Use advanced knowledge and skills in to undertake comprehensive clinical assessments, make decisions based on clinical reasoning and initiate, evaluate and modify a range of interventions

**Heart of the Community**: What Community Services Do

The community virtual ward team will provide care to patients in their own home who would otherwise have stayed in hospital

We have full engagement with our hospital consultants who will retain clinical oversight for their patients on the virtual ward to support the team across each specialist area including respiratory, frailty, heart failure and surgery

Complementing and utilising our existing community therapy and nursing teams, our virtual ward team will provide intensive treatment, care and monitoring to patients requiring
- IV Therapies
- Oxygen and nebuliser titration
- Fluid offloading
- Post-operative management
- To lead the team with management of their caseloads, leading the clinical teams in daily MDT's
- Provide professional senior clinical leadership and support within the specialty multi professional team and across service boundaries
- Lead clinical audits and critically engage in research activity using the results to underpin own practice, inform the practice of others and make organisational recommendations
- The role requires an expert clinician who is a resource for the trainee ACP's.
- To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.
