Job summary

We have the exciting opportunity for Nurse Support Practitioners to come and join our new Complex Transition Support Service

This is a permanent, full-time role, Monday - Friday.

Due to the nature of this role we require a driver with access to their own vehicle.

The service will work closely with North East and North Cumbria (NENC) Integrated Care Board (IBC) Complex Case Management team. The overall aim for CTSS is to support the discharge of people from acute secondary care inpatient beds / private hospital beds into alternative community settings. CTSS will also assist in the repatriation of people back to the NENC region for those currently residing out of area. The service will provide assessments, discharge planning support and 12 weeks post transition support to settle people into their new accommodation.

About the job

As a CTSS Nurse Support Practitioner, you will work with service users and relevant external staff teams to ensure timely, safe and effective transition from acute inpatient areas into community settings.

Main duties of the job

Key Responsibilities

Manage the needs of service users, carers and partner services to ensure delivery (inclusive of all required resident records) meets all relevant quality standards and is person centred, outcomes focused and protective of individual service users dignity and safety. Provide support on assessment completion and support-plan formulation, working with clinical supervisors Prepare for and join MDT reviews to discuss assessment outcomes, facilitate discharge plans and ensure safe transition out to community services. Coordinate with family members / carers as required to involve them in decision making Support and enable internal and external colleagues to meet the needs of service users who express themselves through difficult or challenging behaviour. Ensure that these approaches are linked to nursing support plans. Deliver care and guidance around support plans during the initial 12 weeks of a service user being admitted into a new community placement. Coach and mentor support workers to facilitate their development Undertake any reasonable duties/responsibilities to meet the needs of the organisation.

GCSE Maths and English minimum Grade C (or equivalent qualification). Demonstrable experience of working with people with complex mental health needs Demonstrable experience of working in a community based setting Demonstrable experience of meeting agreed performance targets in a service demonstrating clinical outcomes Demonstrable experience of working with people who have experienced common mental health problems anxiety / depression Demonstrable experience of working with client/patient families and non-professional carers Understanding of complex mental health conditions and behaviours relating to or equivalent experience. Understanding of local and national policies on mental health service provision Understanding of laws, mental capacity act, Mental Health Act, Safeguarding Detailed understanding of medication used in anxiety and depression and other common mental health problems Understanding of anxiety disorders and depression, the stepped care model and high-intensity step-up options to aid clinical decision making in the service and appropriate patient coordination Understanding of the principles of person centred care and overall mental and physical wellbeing Detailed understanding working with people who have complex mental health conditions
