**Role Summary**:
The PBS Practitioner will be part of a team of professionals (Multi-disciplinary Team, MDT) whose role is to support operations teams to deliver a quality person-centred service based on inclusion and meaningful engagement.

**Main Responsibilities**
- You will be responsible for conducting functional assessments, developing Positive Behaviour Support plans and supporting individuals, families and staff to implement a variety of positive behavioural interventions with a person-centred approach.
- Foster a dynamic, inclusive, and progressive culture within the team based on person-centred support with learning environment principles.
- Ensure each individual is supported to maximise their opportunity for dignity, personal choice, positive risk-taking, greater independence, and control over their lives.
- Triage referrals and respond appropriately to each individual’s needs, including, where necessary, gathering and interpreting information to determine eligibility under relevant legislation.
- Work in partnership with other agencies and organisations, including arranging and attending multi-disciplinary /agency meetings to find good solutions for individuals and families, maintaining contact with a wide range of statutory and voluntary sector professionals to facilitate information exchange and connecting people with communities and resources within them.
- Deliver person-centred training sessions using the PROACT-SCIPr-UK framework, and monitor staff training compliance levels.
- Oversee and orchestrate the compliance for management accreditation to deliver PBS training.
- Work alongside the Operations team during the transition of challenging individuals via the PBS Framework and the standard referral pipeline where required.
- Review and analyse incident reports and other documentation to ensure that plans are regularly updated, identifying further needs for adaptation in approach and implementing actions where required.
- Provide evidence of the development of best practice within the services in line with the supported living quality assurance strategy.
- Seek to minimise the potential of abuse or neglect of any individual we support and respond promptly and decisively to manage any such potential risks.
- You must hold a formal qualification in PBS such as Bild CAPBS Coaches or a Psychological and Behavioural Sciences Degree
- You will have excellent communication and influencing skills
- Strong understanding of the values and ethos of positive behaviour support, person centred approaches and thinking, Active Support and Total Communication.
- At least five years of practical experience working with individuals with learning disabilities and complex behaviours of concern is preferred but not essential.

**Job Types**: Full-time, Permanent

**Salary**: £45,000.00 per year
