The post holder is a member of an integrated multi-agency learning disabilities team (Health and Social Services). The team provides a specialist service to adults and older people with learning disabilities in a range of community environments across Camden. Camden is a culturally diverse borough with many multi-ethnic, bilingual and multi-lingual speaking communities. These may provide potential barriers to communication which could affect delivery of client care and the post holder will need to have an awareness of, and sensitivity to, social, cultural and economic issues.

The post holder will take a supporting role to the Positive Behaviour Lead in the implementation of Positive Behaviour Support (PBS) within provider services. S/he will have or develop knowledge of the PBS framework, quality of life model 2 and whole system approaches. S/he will provide advice, guidance, support and training for clients, carers, care staff and other professionals. The post holder will assist the lead PBS worker on the implementation of the PBS framework with a specific focus on Camden’s in houses services as well as leading on other project groups as required for Camden Learning Disabilities Service. S/he will contribute to policy development and will lead on the development of clinical protocols and pathways.
1. In line with plans of care to work effectively as a member of the team to provide a high standard of recovery focussed activity to service users
2. To plan and evaluate groups for the particular service user group(s) using highly developed skills in IT, exercise, gardening and creative activities.
3. To plan and carryout individual assessments and interventions under the supervision of a qualified professional.
4. To be aware of and assist in meeting the psychological, social, physical, recreational and spiritual needs of all service users. To recognise and value service users as individuals, recognising the importance of maintaining the service users dignity at all times. To ensure that the care provided respects equality and diversity that is supportive of service users and their carers.
5. Actively participate in the care and rehabilitation of service users as per the plan of care determined by the multidisciplinary team. Encouraging service users to accept an optimum level of responsibility for their individual programme of care and with their consent, where appropriate, seek the cooperation of friends / relatives / carers.

This role is located in the Camden Learning Disabilities Service (CLDS). CLDS is a fully integrated health and social care multi-disciplinary team for adults and teenagers with learning disabilities either living in the London Borough of Camden (LBC) or for whom Camden maintain responsibility and / or commission services out-of-borough. CLDS are based in LBC premises, but maintains very strong and active links with C&IFT (eg training, professional development, overarching clinical and professional governance).
1. To maintain timely and accurate patient written / electronic records under supervision and in line with Trust and service line policy and procedure
2. To ensure that care is delivered using the recovery approach.
3. To support the assessment and management of patient / service user care needs through initial data collection; monitoring of individual patient progress; feedback; discussion and reports and in care reviews with the team.
4. Contributes towards the ongoing quality of service and care and to identify any risk issues in relation to the service users’ health and social care. Informing the registered practitioner / team / person in charge if there are: -1. any changes in the service users physical or mental state
2. any significant factors relevant to the service user
3. any incidents related to the service users
5. To contribute towards appropriate information to service users relating to health promotion and their well being.
6. To take part in psychological interventions i.e. PBS as per individual plans of care.
7. To recognise and respond appropriately to challenging behaviour in line with Trust policies
8. To understand and follow the Trust risk assessment policy, including being aware of the need to follow all safeguarding procedures; being aware of the necessity to safeguard and promote the welfare of children, young people and vulnerable adults at all times when working with service users.
9. To have an awareness and understanding of Mental Health Law, including the Mental Health Act 1983 (amended 2007), the Community treatment legislation, the Care Programme Approach (1995), and the Mental Capacity Act (2005). Care Act
10. To be aware of personal accountability and responsibility for cleanliness standards and the maintenance of the clinical environment and Trust Infection control policies.
11. To actively contribute to clinical or governance reviews of the teams activity and the monitoring of performance. Contributing to relevant audit practice developmen
