Complex Transition Coordinator

Be a key member of the Care of Children and Young People Steering Group.

Work with a degree of autonomy.

Provide a point of contact for internal and external stakeholders about adolescent and young adult patients with complex needs.

Support the Registered Nurse for CYP to interpret and implement national policies and guidelines on transitions.

Initiate, promote and maintain learning and development opportunities between partner agencies and community groups.

Contribute to reports and briefing papers and assist the Registered Nurse in formulating documents to evidence the work of the transition and young adult care team.

Actively seek the views of children and young people and their carers, using appropriate and accessible methods of engagement. Ensure views are fed into service improvements developments and service re
- design.

Maintain responsibilities in respect of professional practice in line with specific clinical role, Trust policy and procedure and professional background.

Collect data for and contribute to the preparation and writing of reports for key Trust meetings, external meetings, case planning meetings as required and provide assurance to the Trust of robust systems.

Advise, inform and escalate safeguarding concerns to the safeguarding team as appropriate.

Support the Registered Nurse for Children and Young People to ensure the transition from children’s to adult services for young people up to the age of 25yrs with complex needs is managed in a timely and appropriate manner and meets the individual needs of the young person.

Work in partnership with patients/carers, other health providers and partner agencies to coordinate the smooth transition between services.

Assist patients/carers to make choices throughout the transition process and to advocate for their needs during their transition into services at STHFT.

Ensure the principles of the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DOLS) are incorporated into the transition plans of young people

Improve the patient experience of young people with complex needs when receiving care at STHFT.

Work with clinical staff to implement best practice identified through research, national and local guidance.

Collect patient feedback ensuring the service user voice is incorporated into service improvements and re design.

Identify and meet the learning and development needs of healthcare staff in relation to their knowledge of providing services to young people with complex needs.

Develop resources to support staff working with young adults with complex needs.

Evidence improved outcomes for young people with complex needs receiving care at STHFT.
