To work, in SEDS to provide supportive transitions, initial assessments and clinical interventions to people accessing the pathway (age 16+). The post will be based within a Multi-Disciplinary Team. The post holder will work primarily with out-patients to deliver Family Based Treatment (FBT / FT-AN) for eating disorders which includes weight monitoring as part of the intervention. In addition to the above, the post holder will also undertake assessments and develop treatment care packages for service users with eating disorders and their carers, in collaboration with the MDT.

The post holder may also be asked to have a limited role in delivering group work in the SEDS Day Service, which operates three days per week or other ED interventions as highlighted from the assessment of the client. The post holder will work in partnership with service users and other agencies and third sector parties to ensure effective liaison and communication and create effective working relationships. To take sole professional responsibility and exercise autonomous professional and clinical judgement within the overall framework of Trust, and relevant professional registration bodies, policies/guidelines/standards. General requirements This position is for qualified mental health nurses with experience or a special interest in eating disorders.

The job requires an individual who is committed to team working and has the flexibility to work with clients who may be resistant or ambivalent about change on a one-to-one basis as well as in groups. The post holder will work closely with an experienced multi-disciplinary team who will provide ongoing training, supervision and support. The post holder will provide advice and consultation to primary care, mental health teams and other relevant agencies. Principle duties Responsibilities will be split between delivering initial assessments, case load management and primarily family-based treatment (FBT / FT-AN) to service users in the outpatient service.

Other duties include group work interventions, acting as key worker to Day service clients or other ED interventions as highlighted from the assessment of the client. In addition, the post involves liaising with Acute Wards and Specialist Units regarding the needs of patients e.g. patients requiring transitioning into services or admission into these units/wards. Responsibilities include the following: Clinical To provide a specialist clinical service in eating disorders, including initial assessment and managing an agreed caseload.

**This will include**: Participating in daily triage of referrals with other Team members where referrals are screened for suitability for assessment by SEDS or referral on to the most appropriate treatment in line with care pathway. Undertaking specialist initial assessments, including risk assessment as part of the Scheduled Care Pathway. Reporting back on the outcome of assessments to the MDT and developing treatment packages based on a collaborative formulation of the clients needs. Delivering evidence-based treatments including monitoring risk, providing psycho education and engaging and motivating clients in working towards agreed treatment goals.

Ensure that all clients have a clearly defined and agreed collaborative care plan, designed to meet their needs and to provide the agreed intervention. Monitoring the physical health and wellbeing of clients with eating disorders and liaising where appropriate with primary care and in-house monitoring. Collecting relevant data e.g. PROMS to evaluate and review treatment at appropriate intervals using both clients self report and service outcome measures.

Ensure all clients receive appropriate follow up and relapse prevention. Working in partnership with service users and their relatives providing support where appropriate to ensure their engagement in treatment. Case management and liaison with services/clients and any other agencies for those CYP who require transition between services. Providing progress reports and maintaining effective communication and liaison with referrers, General Practitioners and other relevant professionals/agencies including in patient areas.

Maintaining appropriate health records as per the service and Trust policy. Managing a caseload in line with relevant policies, using strategies to minimise unplanned activity. Working with clients who have eating disorders and who require ongoing support to manage their needs. Attend and participate in multidisciplinary team meetings and training sessions as required and directed.

Support into wards where patients are admitted for an acute episode of care To participate in clinical and Care Planning meetings outside of the Eating Disorders Service to provide specialist advice and facilitate cross-referrals.
