To coordinate and supervise a shift, communicating effectively with members of a multidisciplinary team to deliver high quality, evidence-led care to patients.

To exercise clinical judgment and decision making about patient needs in terms of long term goals or plans.

To participate fully in the day to day organization and running of the Vascular Access Team including the triage of referrals, PICC insertion, troubleshooting and monitoring of patients with CVADs in situ and liaison as required with other medical teams.

To assume personal accountability for nursing care delivered to a patient in accordance with the NMC code of conduct. There is an expectation for post holders:

- To promote a safe clinical environment for all patients, visitors and staff
- To participate in Trust initiatives around infection control and care quality indicators
- To assist with any local or trust initiatives to ensure the continuous improvement of the quality of services and safeguarding of high standards of care
- To be aware of all hospital policies and procedures and collaborate with other health care professionals to ensure that these are observed

The Vascular Access Specialist Nurse will coordinate the PICC service in Sutton/London and triage referrals for all type of CVADs across the Trust. They will undertake assessment for, insertion and removal of PICCs, STCs and PORTs. They will be involved in teaching, audit and product evaluation.

Be expert in peripheral intravenous cannulation.

Be expert in the insertion and removal of Peripherally Inserted Central Catheters (PICCs).

Undertake training and become proficient in utilising ultrasound guided venous cannulation techniques (e.g. peripheral and central veins). Use ultrasound to identify common anatomical variations (e.g. thrombosis of target vein).

Be able to verify tip catheter position and recognition of complications such as pneumothorax using image intensification, ECG technology and tracking devices. Ordering and interpreting radiological investigations such as CXR when necessary.

Be able to recognise and manage complications during and post insertion of CVADs Undertake training (if required) to acquire expertise in the insertion and removal of other type of CVADs (Acute CVCs and STCs) utilising standard percutaneous approaches (i.e. internal jugular, subclavian/ axillary and femoral veins).

Cross cover for colleagues across sites as required, to ensure consistency of service provision is maintained.

Coordinate the vascular access central referral system, triaging referrals as clinically indicated and treatment requirements. Use specialist knowledge and skills to assess clinical need and risk and communicate with the referring teams any varying autonomously-made clinical decisions that are deemed appropriate and/or necessary.

Work collaboratively with the Lead Vascular Access Nurse in the development of protocols and guidelines to support a nurse led vascular access service.
