An exciting opportunity has arisen for an Assistant Patient Pathway Coordinator in the Department of Chronic Pain CSU. You will have the unique opportunity to support and gain experience within the Specialist Services.

The post holder will provide a pivotal administrative service to consultants and their clinical teams. They will work flexibly with the other team members within the Clinical Support Unit and provide a professional, efficient administrative service to patients. In addition they will be expected to be able to provide scheduling support when required, including the booking of outpatient activity and waiting list management. This post is 1.0 WTE to cover Monday to Friday and the working hours are 9am-5pm.

This role will be working alongside the Patient Pathway Coordinators within the Chronic Pain Team. All staff are supported with their training needs and regularly share learning opportunities within the department.

Job Purpose
The post holder will act as a point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support to the Patient Pathway Co-ordinators (PPC) and Patient Pathway Lead (PPL) in the
proactive management of the patient pathway from referral to discharge, ensuring the entire
pathway is managed smoothly.
The post holder will ensure high levels of patient and clinician satisfaction by being an
accessible, customer focussed and knowledgeable point of contact.
Main

**Responsibilities**:
Outpatient Pathway
- Process and log all referrals, including paper and Choose and Book, in line with

Trust Access Policy and speciality-specific Standard Operating Procedures
(SOP).
- Schedule new and follow up appointments with the most appropriate clinician,

ensuring that capacity is proactively and efficiently used, escalating upwards
where this cannot be achieved.
- Ensure that any diagnostics, investigations and additional information that are

required as part of the patient pathway have been requested, including those
carried out at other Trusts.
- Assist in the co-ordination of appointments and procedures at other hospitals and

organisations, where required.
- Ensure clinic outcomes are recorded accurately on PAS, including those for offsite

clinics.
- Process clinic outcome forms, according to SOP.
- Reschedule outpatient clinics as requested, in line with waiting time targets.
- Liaise with health records staff, ward clerks and other organisations to ensure all

medical notes, referral letters, results and discharge summaries are available for
outpatient appointments and consultations.
Pathway Tracking
- To use the 18weeks referral to treatment (RTT) to manage all outpatient and elective

patient journeys.
- Assist in the validation of the RTT Patient Tracking List (PTL). Investigate and take

appropriate action, under direction, where pathways are incomplete to ensure that
patients are receiving timely treatment and that Trust Information is robust.
- Support the identification and escalation of any issues to the PPC which compromises

delivery of the 18week RTT pathway, e.g. lack of capacity either in outpatients or
theatres.
- Ensure inter-provider transfers are timely and that the appropriate paperwork has been

completed and sent or received.
- Liaise with internal and external colleagues to share patient pathway and diagnostic

information.
- Manage and monitor outpatient and theatre scheduling making best use of capacity and

escalate to PPC where this cannot be achieved.
- Ensure Trust systems are updated with patient pathway status information and that data

quality is maintained.
- Proactively manage pathways to avoid breaches and take steps to resolve any issues,

escalating where necessary.
Inpatient Pathway (Speciality Specific)
- Ensure that Trust agreed standard of service is delivered to patients requiring

elective adm
