An exciting opportunity has arisen for a Patient Tracker on Bay Tree which is a 30 bedded acute surgical ward at Hinchinbrook Hospital. This role is fundamental in supporting patient flow throughout the trust and maximising bed capacity.

We are looking for an enthusiastic, driven and motivated member of staff who has great communication skills, to complement our existing team on Bay Tree.

The main focus of this role will include assisting in patient discharges, liaising with other departments, and making a valued contribution to the overall patient experience.

You will role model expected clinical behaviours and ensure discharge planning is carried out to a consistent high standard.

The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care. The post holder will provide a single point of contact for the management of bed availability and allocation within ward. The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and ‘real time’ understanding of the ward’s current and predicted capacity position and the timely and well planned discharge process.
