We are looking for a motivated Band 4 Family Support Worker to join the Palliative and Complex Care Team to provide essential respite to children currently on the team caseload.

The post holder will work as part of the PACCT Community Nursing Team, with support from the band 5/6’s and the Team Leader. You will be involved in providing care in children’s homes, in community settings, or even within the acute trust (RSUH) for children with largely palliative conditions. The PACCT team supports the Staffordshire ICB with a clear remit of named nurse caseload working, providing ongoing support of children and families, being an advocate for service users, and offering an end-of-life service to enable children to remain at home if this is their wish.

We have a busy and culturally diverse caseload of children and young people with palliative and complex health needs. We also support our acute service, with support involving complex care and palliative care. We also have robust links with multidisciplinary health services and with education and social care.

The main duty for the post holder will be to provide short-break respite for service users, mainly within the home environment.

You will support registered practitioners to assess service users, and contribute to the development and implementation of programmes of treatment/ care/support for children, young people, their families and carers. The care/support plans may include the following:

- Promoting independence/self-management and maintaining safety
- Provide short breaks and/or support to children and their families with complex heath care needs
- Increased functional ability
- Improvement in health and wellbeing including mental health
- Interventions in disease/condition management
- Prevention and reducing of health inequalities
- Admission avoidance
- Early supported discharge
- Contribute to case management/ treatment /care plans
- Palliative and end of life care

You will use your own knowledge and judgement to consider a range of care and support packages that are available to service users. We always consider the personal circumstances or wishes of our Children and their families, and will adjust any programme to support these wishes, whilst ensuring maximum service provision and care.

Any adjustments made to programmes of care and intervention will be reported back to the registered practitioner.
