The Active Recovery department are looking to recruit a new Team Administrative Support Assistant.

Active Recovery is a multi
- disciplinary team providing clinical support and rehabilitation to patients at home. The focus of the team is to promote admission avoidance into hospital, facilitate early discharge from hospital and to provide an urgent response to patients that may be in crisis at home. Active Recovery is a city-wide service which operates 7 days a week, 365 days per year.

The Post holder will be expected to communicate with a significant cross section of senior staff within STHFT, and external organisations, often dealing with confidential and urgent enquiries and correspondence.
Undertake any other duties which may be allocated within the general scope of the post and appropriate to the grade.

To ensure all ACSW have reported in for the start of their shift.

To check that all patient visits have been allocated to a CSW by cross checking records in Microsoft Excel.

Live maintenance of ACSW Rotas with staff sickness, training, meetings etc.

Monitor Tasks on SystmOne and action them or escalate as appropriate.

To support the OSC team to maintain staffing levels through overtime and bank etc.

To update patient and staff information as directed by an OSC or Planner including adding/ deleting patient visits and discharging patients.

To take minutes and record action points for team meetings.

Communicate complex, sensitive and contentious patient information effectively with patients, carers, and staff in the multi-disciplinary team and other agencies via telephone and face to face.

To prepare weekly reports for senior managers using pre-existing templates.

To undertake any other duties that might be considered appropriate by the Operational Business Manager after negotiation and adequate training.
