The role of the discharge co-ordinator is to help the flow of patients in and out of the ward. The main duties is to help with complex discharges on the ward and to liase with the nurse in charge.
Main duties of the job
This is an exciting opportunity for a secondment Band 3 discharge co-ordinator to join a diverse and motivated team that is dedicated to the development of skills and the delivery of high quality safe care. We pride ourselves on maintaining flow and providing an efficient and quality service, and the discharge co-ordinator plays a large part in this.
We have a management team that is both innovative and proactive in ensuring that high quality care is delivered to our patients. We are keen to encourage developments in line with Trust policy and working in new ways. We recognise responsibility to ensure that staff are developed and stimulated within their environment in order to achieve vision and deliver care that reflects the values and behaviours of the Trust.
Working for our organisation

They will work as part of a MDT team and there will be some independent working.
Excellent communication skills is a must.

The post holder will have training provided prior to starting their new post. The post holder will shadow an experienced discharge coordinator for some supernumerary time to help them transition into this new role.

Essential criteria
- Evidence of a good standard of written and spoken English
- Educated to GCSE level
Experience

- Demonstrate understanding of patient/client confidentiality
- Experience in delivering excellent customer care
- Experience of effectively working with a team of people from different professions/backgrounds

- Previous experience of working in healthcare

**Salary**: The quoted salary will be on a pro rata basis for part time workers.
