The post offers a unique opportunity to be a key leader of change in the drive for parity for those people’s lives affected by dementia within Lincolnshire. We are looking for an individual able to work across the system stakeholders to foster and enable system wide collaboration, with a focus of person-centred, place-based care in pathway alignment and the development of key service delivery milestones. To support this exciting and important piece of work we need a highly motivated, dedicated, and passionate individual, with experience of working at a senior level with system leaders across, health, social and third sector, along with an extensive understanding of transformation and projects management and delivery.

To provide day to day management, oversight, and delivery of the dementia transformation programme (as falls under the Lincolnshire Dementia Programme Board (DPB)). Working at a senior level with system leaders across health, social care, ICB and VCSE, to support the DPB SRO and associated senior managers and engaging stakeholders (including staff and service users) to develop and support agreed delivery of a Countywide dementia strategy; for people of all age experiencing dementia.

- Lead on Dementia Programme Board aligned strategy and service development and delivery
- Ensure all elements of agreed development and implementation are undertaken
- Work with corporate and system departments to ensure reporting, finance, contracting, workforce and engagement are properly implemented.
- Support Operational Associate Director/DPB SRO and organisational service managers/heads to Implement change within their respective areas.
- To work with system partners to develop and implement dementia pathway change across the system.
- To ensure appropriate, timely and accurate reporting within the Governance requirements
- Ensure that effective operational management and governance arrangements are in place for transformed services/service pathways.
- To ensure that programme board transformation objectives are met to ensure effective co-production, social inclusion and partnership working arrangements.
- To contribute to financial planning processes as part of the Trust’s and broader ICS annual planning arrangements.
- Ensure a competent workforce through the recruitment, management and development of staff
- Promote wide partnership working where required.
- Lead the design of care pathways for dementia services delivery ensuring focus on personalised services
- Ensure effective HR and Learning and Development practice where required
- Produce progress reports for Board, commissioners and other stakeholders as required.
