**Contract**:This is a 12-month contract, with the potential for extension.

**About the role**

We have an exciting opportunity for a Dementia Adviser to fulfil a new role in North Lincolnshire, you will support people, who are concerned about their brain health, enabling them to engage with their GP to explore their concerns further and provide ongoing support during referral to the Memory Service. You will be part of a highly motivated and supportive team.

We are seeking a passionate confident individual to lead on promoting and creating awareness of brain health. In this role you will demystify the memory assessment process and promote the benefits of seeking a diagnosis.

Relationship building with local communities and religious groups will be a key element to this post. You will establish Brain Health pop up information stands within community spaces, like libraries, where you will be meeting the public, providing information and advice on:

- Understanding memory problems and strategies for coping with them.
- Discussing memory concerns with their GP and what this is likely to involve.
- Seeking assessment and diagnosis, including the different steps toward receiving a diagnosis and how to overcome any barriers that they have encountered.
- Understanding the roles of different health and social care professionals who may be involved in the diagnosis process.
- The roles of other services who can provide support for mental wellbeing throughout the assessment process and how to access them.
- What will happen after a dementia diagnosis, support available and how it is accessed.
The role will also involve creating strong working relationships with primary and secondary care and community services.

**About you**
- You are a natural influencer and have the ability to quickly build a rapport with people creating a safe space, that will enable them to discuss mental health concerns.
- An understanding of delivering a service using the person-centred approach.
- Understand client confidentiality and how this is applied when representing client needs.
- Have some previous experience of making local contacts and networking with various services and providers.
- Have demonstrable experience and understanding of dementia and the ability to communicate that in a clear and relatable manner.
- Demonstrate a non-judgmental approach, be empathetic and have a commitment to equal opportunities.
- A broad understanding of statutory and voluntary services.
- Comfortable connecting with others and talking to members of the public and a wide range of other health care professionals.
- Must be able to travel independently across North Lincolnshire and have access to own vehicle.

- Excellent communication skills, both verbal and written.
- Excellent organisational and timekeeping skills.
- Excellent attention to detail.
- Work collaboratively and be able to prioritise your workload effectively and remain solutions focussed.
- Strong interpersonal and interpretation skills.
- Be a self-starter who is proactive, creative, a good listener and incredibly motivated.
