To support the integrated working of our health and social care teams to ensure people receive joined up and effective services.
- The central co-ordination point for the local multi-disciplinary activity to include intermediate care, community therapy, community nursing and social care clients and other clients requiring support who are known to health or social care.
- To be a central co-ordination point for the local multi-disciplinary teams including both health and social care. Facilitation and coordination of regular multi-disciplinary meetings and ensure follow through any actions relating to client discussion
- Liaison with health and social care colleagues in the local community including voluntary sector representatives to try to aid the safe and independent living of complex clients in the community
- To support health and social care team colleagues in the utilization of a range of ‘case finding tools’. These tools will help identify people at risk of loss of independence, potential risk of unnecessary admission to hospital or longer term care.
- To be responsible for a high quality customer service function in recording contact information, supporting the initial prioritization of contacts, subsequent feedback and on-going liaison with referrers and relevant others.
- To maintain IT based information systems and take a key responsibility for the production of key performance data.
- To contribute to 7 day working rota over a 8-6pm service weekdays and 9-5pm at weekends.

This post is the gateway to the Moor2Sea services. A multi-disciplinary team that includes physiotherapists, occupational therapists, nurses, assistant practitioners, support workers, coordinators, social care assessors and social workers.

The coordinators are the main and first contact for all referrals to the locality whether under community therapy, Intermediate care, community nursing and or social care.

There are plans for service developments to enable a single point of contact phone line to facilitate this even further but at present this referrals can come in through a variety of ways and services.
