**Project Support Officer**

**Location**: Bedfordshire, Luton and Milton Keynes, administrative base at Head Office, Bedford

**Salary**: £22,604 - £25,796 annually

**Hours**: 37.5 hours per week

**Line Manager**:Operational Lead - Keyworker Team

The Key Worker Pilot is part of the NHS long Term Plan for All Children and Young People (CYP) with a learning disability, autism or both, in an inpatient setting, to have a named worker/Key Worker by 2024.

**The vision is**:

- Children and young people with a learning disability, autism or both with the most complex needs will have a designated Keyworker.

**The mission is**:
To facilitate the development and implementation of a Keyworking function at a local level by:

- Developing a cross‐system function, with ability to build relationships and access resources from across the system.
- Ensuring an appropriate level of provision is in place and accessible to the children and young people who need it, when they need it.
- Embedding a support service to manage crises and support community living.
- Improving the use of data from dynamic support registers to ensure timely identification of young people for support and to inform the commissioning of preventative services.

**Job Summary**:
The post holder will assist the Keyworker Team in coordinating community services to support those children and young people who are at risk of hospital admission due to their deteriorating mental health. This role will also assist in organising and facilitating a range of specialised and tailored support services that families can access to improve their lives. The Project Support Officer will be required to confidently engaged with young people and play an active role in our farm project.

The post also requires a high level of organisational skills and multi-tasking to ensure that the project runs smoothly and that there are clear lines of communication with families, colleagues and funders.

**MAIN TASKS;**

**1.** **To support the identification of required support**
- Identify what current services and support the young people are accessing.
- Identify what services and support they feel would benefit them and their family.
- Begin to co-produce and tailored an appropriate family support plan where required.

**2.** **To provide ongoing communication with identified families**
- Assess and monitor the family’s needs.
- Share appropriate and useful information with families regarding local support and opportunities.
- Record all correspondence accurately for monitoring and evaluating.
- Liaise with statutory services around any developments of family need or support.

**3.** **To organise, coordinate and deliver bespoke services and support**
- Research appropriate opportunities/support that young people could access to improve their health.
- Support the identification of need based on feedback from the children and young people.
- Where opportunities occur outside of Autism Bedfordshire services, liaise with providers to ensure successful and supportive experiences can occur.
- Support the young person to access opportunities where required.

**4.** **To provide support around 1:1 personalised care and support planning**
- Meet with family’s, Children and Young People, assess their care and support needs.
- Create goals, plans and visual resources using a range of software and design tools.
- Facilitate opportunities for young people to access support.
- Monitor, evaluate and report to the Key Worker Team

**5.** **To attend multi-disciplinary meetings where necessary**
- Update Keyworker team on progress
- Liaise with colleagues from education, health and social care where appropriate to enhance family support.
- Discuss any needs for additional or alternative services or support.

**6.** **Evaluation, progress and outcomes**
- To keep accurate data and narrative records of communication and progress
- Working closely with the Keyworker Team to review caseload, feedback and evaluate progress.
- Support the Children’s Services team to compile detailed reports and evaluations relating to all elements of the project.
- Support project meetings/presentations with health colleagues when required, to demonstrate impact and outcomes.
