We have an exciting opportunity for a part-time (30 hours per week) enthusiastic and dynamic Band 6 Care Coordinator to join the Early Intervention Service. Working in our newly developed Early Detection Pathway called EDIE, you will form part of a Multi-Disciplinary Team providing support and interventions to young people aged 14 - 35 years old who are assessed as having an At Risk Mental State (ARMS) for developing psychosis. Our EDIE (Early Detection Interventions and Engagement) service offers specialist assessment, advice and a range of supportive, therapeutic interventions to our service users and their families; aiming to prevent or delay the onset of psychosis and facilitate access to recommended treatment interventions at the earliest opportunity. The service also works to support education, understanding and early identification of psychosis and ARMS, reducing stigma and supporting early access to support.
To have the responsibility for the delivery of an Early Intervention in Psychosis Service to specific individuals on the ARMS Pathway.
Coordinating clinical activity, liaison with the rest of the Early Intervention in Psychosis Service and other appropriate teams, services and agencies.
Immediate problem solving and decision making and contributing to long term planning for future developments both within the immediate clinical area and within the service area.
To ensure that the values and principles of the organisation and of the service area are translated into the philosophy and operational policy of the clinical team and that these are evident in everyday practice.
To contribute to the instilling of the Early Intervention in Psychosis Service values that promote the rights of the individual and wherever possible aim to protect the autonomy of the service user and involve them in their care.

To ensure that new policy legislation, recommendations and guidelines are effectively communicated, understood and integrated into daily practice. (See appendix for examples of the same)
Maintain working practices that promote interagency and multidisciplinary working and ensure effective and accurate communication with particular emphasis upon inpatient facilities and the interface between inpatient and community care. Attend appropriate meetings / forums to ensure that the wider clinical team (multi-disciplinary team and locality team) are fully engaged in both clinical and developmental aspects of the area.
Take responsibility for maintaining professional registration and personal and professional development in accordance with NMC guidance, attending statutory and mandatory training and other pertinent training as agreed with the Team Leader through the personal review process.
To contribute to the strategy and clinical governance plans for the Early Intervention in Psychosis Service and the wider service area to which the community nursing team belongs. Take a lead in implementing specific projects in relation to this. Develop and maintain links and support networks with other teams within the service area, ensuring consistency across the service.
