Lanarkshire, United Kingdom NHS Scotland Full time

Band 4 £21,658 _ £24,297

1 x full time (35 hours) covering North Lanarkshire & Clydesdale using digital technology

(Project anticipated length 3 years)

Do you have excellent communication skills and strive to provide a high standard of care to clients and carers including undertaking assessment, planning and implementation of action plans. Could you visit clients and carers in person or via digital technology and offer emotional support and help signposting and facilitating access to appropriate care and support? Could you help recruit, train and support our Volunteer Befrienders? If so you may be just who we are looking for

Compassionate Lanarkshire (CLAN) is a pan Lanarkshire project jointly funded by National Lottery Community Fund and South Lanarkshire Health and Social Care Partnership which aims to support vulnerable and isolated individuals and families living in Lanarkshire who are affected by life limiting illnesses including cancer, chronic obstructive pulmonary disease, heart disease and progressive neurological conditions. CLAN is a partnership project between St Andrew’s, Kilbryde and Strathcarron Hospices.

CLAN will offer emotional, therapeutic and practical support to help individuals and families identify, value and build on their strengths to achieve the best possible quality of life. CLAN will work with service users (clients and carers) in their own homes and communities to identify and build on their strengths, increase knowledge and understanding, manage stress and anxiety, enhance resilience, improve emotional health and wellbeing, identify and access services, supports and opportunities: all with the aim of reducing isolation, loneliness and anticipatory grief.

This post is funded by National Lottery Community Fund and South Lanarkshire Health and Social Care Partnership
