The post holder will work as an integral part of the Community Cardiac Specialist Team to provide ongoing care and monitoring for patients with diagnosed cardiac conditions. Supporting patient self-management strategies and assisting in the prevention of unnecessary admission to hospital.

The role will include monitoring and supporting patients who attend community clinics and cardiac rehabilitation programmes, visiting patients in their own homes, and providing telephone support and advice.

The post holder will require good clinical skills appropriate for the role and will be an excellent communicator.

Some administrative skills will be required which will involve maintaining computer records and ensuring a high standard of record keeping.
- To work under supervision of a qualified nurse to assess, review, monitor and support patients with cardiac conditions according to evidence based guidelines.
- To be accountable for all aspects of own work and to manage the workload efficiently and effectively with regard to clinical priorities and use of time. To give patient centric care as per individualised plan ensuring the highest possible standard.
- To provide evidence based therapeutic interventions as per individualised care plan and evaluate care interventions
- To document actions and findings in patients’ notes in accordance with local and national standards.
- To report to the senior nursing staff.
- To participate in the Cardiac Rehabilitation Programme and assist patients with their physical exercise regimes as directed by the senior cardiac specialist nurse.
- To be responsible for safe and responsible use of equipment as per policy/ guidelines.
- To adhere to local and national infection control policy.
- To respond to clinical incidents in the appropriate manner according to organisational policy.
- To support the collection of statistical data and partake in statistical review of the service.

What is Staff Bank?

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

- To accept and act on duties delegated by the registered nurse
- Provide care and support for patients who have various cardiac conditions such as myocardial infarction, undergone a cardiac revascularisation procedure/surgery, have a diagnosis of heart failure ( list not exhaustive)
- To work autonomously to obtain patient consent and perform a general cardiac assessment of patients to include;
Blood pressure, Heart Rate, Respiratory Rate, Temperature, Pulse oximetry,

Skin Observations, Hydration (Weight/ fluid balance)

Phlebotomy - whilst not essential would be a desirable skill
- To take a simple history of presenting complaint and progress.
- To autonomously progress patients through prescriptive exercises as per protocol and based on patients individual assessment results and ability
- To have a practical understanding of therapeutic handling techniques to facilitate safe practices within cardiac rehabilitation.
- To monitor patient progress reporting changes and seeking guidance regarding further treatment from the senior cardiac nurse without direct supervision.
- To participate as an active member of the multi-disciplinary team, reporting relevant information to other members of the team
- To exhibit a professional attitude and maintain a high standard of competence in the skills taught, observing the rules of confidentiality and respecting the rights of patients.
- To be able to signpost to appropriate MDT team members when required.

COMMUNICATION
- Communication with patients and carers is central to the role and the post holder will be a good communicator both in person and on the telephone, and will be able to give appropriate health and lifestyle advice in line with evidence based guidelines.
