THE MERRYWOOD PRACTICE

**1.0 JOB DETAILS**

Title: Care Co-ordinator/Medical Receptionist

**Salary**: Starting at £10.90 per hour

Hours: 15 - 20 hours worked on a flexible rota

Location: William Budd Health Centre

**2.0 JOB PURPOSE**

To provide an efficient and effective Reception and Medical Records service to patients

and doctors.

**3.0 RESPONSIBILITIES**

3.1 The postholder is directly responsible to the Reception Co-ordinator

3.2 The postholder is not responsible for any other staff

**3.0 KEY TASKS**

3.1 To register patients on the computer system as having arrived for their appointments.

3.2 To follow practice protocols and pathways to ensure consistency of service.

3.3 To make future appointments on the computer system for patients following agreed pathways and protocols.

3.4 To ask patients agreed questions and signpost to alternative services.

3.5 To issue repeat prescriptions using the clinical computer software and pass them to appropriate clinician for electronic signing.

3.6 To act as the initial contact for all persons entering the Health Centre, including contractors and maintenance staff.

3.7 To register new and temporary patients when necessary.

3.8 To answer all telephone enquiries.

3.9 To scan and download documents into patient records.

3.10 To respond to all ad hoc requests for assistance from the Doctors or reception co-ordinator

3.11 To contact patients at the request of the doctors.

3.12 To monitor the practice e mail account and respond to queries in line with agreed processes.

3.13 In the absence of the reception co-ordinator, to allocate test results and prescriptions to doctors.

3.14 In the absence of the care co-ordinator, undertake routine searches for patients requiring contraceptive & HRT checks.

3.15 To liaise with other health care workers as necessary.

**4.0 HEALTH AND SAFETY**

Under the provisions contained in the Health and Safety at Work Act 1974, it is the duty of every employee to:

2 to co-operate with the Trust, as far as necessary, to enable them to carry out their legal duty, and

3 not to intentionally or recklessly interfere with anything provided, including personal protective equipment, for health and safety and welfare at work.

**5. CONFIDENTIALITY**

All information concerning the Practice’s patients and staff must be treated as strictly confidential at all times, and you may not divulge it to any other person except with the authority of the Practice Manager or one of the Partners. Such authority may only be given when it is the patient’s or staff’s interest and is a necessary part of treatment.

Unauthorised disclosure of confidential information will result in disciplinary action and may lead to your dismissal.

**6. TERMS AND CONDITIONS**

The post holder will be entitled to join the NHS Pension Scheme

Your job title is Care co-ordinator and this job description sets out the duties which such a job normally entails. The nature of our business means that you must be flexible in your approach to your work in order to provide service to our patients.

Essential Requirements

(i) Ability to work under pressure, and in a sometimes challenging environment

(ii) Excellent communication skills, written and spoken

(iii) Aware of patient confidentiality

(iv) Prepared to be flexible with time and duties

(v) Able to work as part of a team

(vi) Experience of working with different IT systems and packages

Desirable Requirements

(i) Experience of clerical work

(ii) Experience in a health care setting
